Care provided in patient's own environment i.e. home, assisted living, group home or a nursing home. 

Routine home care includes but is not limited to: 

• Nursing visits
• Social worker assistance with community resources 
• Chaplin provided spiritual support
• Volunteers to give relief to caregiver
• Aide to help with personal care
• Bereavement Services 
• A physician and nurse are on call 24 hours a day 7 days a week

Continuous care provided in patient's environment on a short-term basis when symptoms are not controlled at the routine home care level. 

Care provided in the hospital or long-term facility on a short-term basis for pain and symptom management. 

Care provided in a facility to give caregiver/family some relief.
